v0.4.0 -- New name, new simpler role

Now known simple as 'report', work and simple are both retired into a single report template. The other plans for this have been abandoned. If I approach them again, they will be their own package.

The difference between the two is now handled by setting title_page: true, turning on the title page and table of contents.
