Fix MCP startup for hosts that send server/discover before initialize. Empty probes now receive an error response without closing the connection, so initialization can continue. Metadata-bearing discovery requests also work.

The server uses rmcp 3.5.1. Regression tests cover both lifecycles. Both paths were verified with tool discovery and a real doctor call.
