0.5.4 release repair

This release completes the stable promotion that 0.5.3 never finished.

• Restores @arcships/light-ocr to the stable image OCR + doctor surface and preserves its script-free, offline-installable closure.
• Ships PDF and multi-page OCR as the explicit @arcships/light-ocr-document@next preview with its own light-ocr-document command.
• Removes hostname-derived stable identifiers from voluntary diagnostics.
• Adds real PDF rendering/OCR, DPI-aware resource limits, cancellation, deterministic cleanup, TypeScript and CLI contracts.
• Tests stable offline install and real Document PDF OCR on macOS arm64/x64, Linux arm64/x64, and Windows arm64/x64.
• Corrects README and release records that had described the next-only 0.5.3 candidate as complete.
