Fixes

• Fixed-size embeds stay fixed when edited. A video stored with Responsive switched off used to load with it switched back on, and saving then marked it responsive again. The toggle now keeps the stored value. New and empty videos still default to responsive.
• Fixed-size embeds render at their stored size. <x-matinee::embed> used to stretch every iframe to the full width. It now does that only for responsive embeds, so a fixed-size embed renders at its stored pixel dimensions, as documented.
• Missing sizes fall back to 16:9. A stored video without a width or height raised an "undefined array key" warning and rendered with a size of 0.
