macOS settings and permissions

Agent CLI's macOS app now has a resizable settings window with a sidebar for General, Recording, Shortcuts, Permissions, and Advanced. Recording options, model settings, keyboard shortcuts, and diagnostics are easier to find, with help beside the controls it explains.

A dedicated Permissions page shows microphone, Accessibility, and notification access. Permission checks are silent; requests happen when you choose them. Starting a recording without microphone access opens setup first. Denied access opens the relevant System Settings page, and status refreshes when you return. Notifications remain optional, and clipboard recording works without Accessibility.

Accessibility recovery now requires confirmation, is unavailable during active work, and reopens setup after restart. Existing preferences and shortcuts are preserved.
