macOS voice setup fixes

• Show the actual first-use setup stage, including runtime installation, voice-service startup, and speech-model preparation, with elapsed time and a clear “Not recording” label.
• Cancel a pending hold-to-transcribe recording when you release the shortcut before setup finishes. Setup continues in the background, and the app tells you when it is ready to try again.
• Move the setup card and recording/transcribing indicator by dragging them, or minimize them to the menu bar. Restore them with Show Voice Setup… or Show Voice Activity…; progress updates and recording attempts preserve minimized state.
• Report voice-service port conflicts promptly and avoid saving a ready marker before startup checks succeed.
• Prepare the model again after failed setup or a service reinstall. Unrelated commands no longer replace failed voice setup with a misleading ready message.
