Version bump 4.0.1 → 4.0.2 with changelog entry covering the severity
vocabulary restore (`advisory` → `high`), the fifteen rules returned to
`high`, the severity-ledger conformance guard, the loop-query queue-consumer
discriminator, the corpus re-pin, and Spec 60 reporting metadata.
