• feat(ir): persist the Scraps IR under .scraps/ and read every command through it (#721)

  The IR the pipeline diagram always promised is now a file: .scraps/ir.json next to .scraps.toml holds one object per source (identity, content hash, and every parsed fact) plus the link table, with unresolved references kept as resolved: false edges. Every CLI command and MCP tool reads the wiki through it, so an unchanged source is never parsed twice, and build --git reuses commit timestamps while HEAD is unchanged — on a thousand-scrap wiki that takes a warm --git build from about 3.4 s to about 0.55 s. The directory ignores itself in git, is rebuilt from source when its version or content no longer matches, and never fails a command. See the new IR reference page.
