Fixes

• unified: stop hunk body at lines not starting with space, +, -, or \. Surrounding metadata (diff --git, index, Binary files, free text) is no longer silently absorbed into the previous hunk as context, which previously inflated hunk line counts. (#62)
• unified: terminate hunk on bare blank lines instead of treating them as empty context.
