fix: make server lifecycle more test-safe by tracking background threads

All fire-and-forget daemon threads (startup-refresh, stats-refresh-*)
are now tracked, gated on the shutdown event, and joined during lifespan
teardown. Prevents threads from leaking across TestClient contexts and
touching a reset DB connection.

fix: cache endpoint returns zero results for drive-root scans

When scanning a Windows drive root (D:\), the normalized root is "/".
The LIKE pattern became "//%" (root + "/%"), matching nothing.
Now uses "/%" directly when root is "/".
