Ideally, we would have used the caDSR-on-FHIR service, but that appears to be down at present.

This PR introduces a workflow for harmonizing data using the caDSR (as accessed through the caDSR-on-FHIR service). The workflow is a little convoluted, but explained in the included README file. This PR includes support for exporting harmonized data as CSV; later PRs will add support for exporting as PFB, CEDAR instances and as JSON-LD.
