Reader (@chaoxu/coflat/reader)

• renderToHtml opt-in outline — emits a stable id on every heading (explicit Pandoc {#id}, else a diacritic-folded deduplicated slug) and returns a ReaderOutlineEntry[] document outline (id, text, level, canonical number). Default output is unchanged. (#35)
• Reference catalog line — buildReferenceCatalog / analyzeReferences in @chaoxu/coflat/parse now expose a 1-based line (full-source) on every target and reference. (#36)
