Trace and usage (#28, part of #23)

• Trace records a stop event per turn with its index, per-turn and running token totals, and cache tokens.
• Stop reasons are normalized. The per-provider mapping is documented. content_filter and refusal stops map to error.
• Usage now includes cache-creation and cache-read tokens for Anthropic, OpenAI, and Gemini.

Files changed (#26)

• New files_changed field, measured from run start (also after resume), excluding loom trace files.
• A truncated flag is set when there are more than 1000 entries.

Models (#27)

• An unknown or unserved model id fails on the first call with a one-line message, exit code 2, model_not_found, and no retry.
• New loom models --provider P [--json] lists model ids (ollama, openai, anthropic, gemini).
• capabilities now lists subcommands and default_models.

Parallel runs (#29)

• A second loom process on the same --workspace or --trace is refused with exit code 2. Locks are Unix only.
