Kotlin files now show up on the map.

• Kotlin joins the file graph. OpenLore now reads the package, imports, and package-level
  declarations of .kt and .kts files. Before, a Kotlin file had no imports, no exports, and no
  import edges. Imports resolve by declared package, not by directory, and Kotlin and Java files
  resolve each other. A wildcard import reaches only the files whose names the importer uses; a
  name declared in two files reaches nothing and is counted as ambiguous, never guessed. Exports
  follow visibility: public is listed, internal is tagged, private is left out. (#546)
• An unreadable index says so. When the index exists but cannot be served, MCP tools now
  diagnose the cause instead of reporting that no analysis was found.

Also cleared dependency advisories in proxy-addr, sharp, source-map-js, fast-uri,
brace-expansion, and braces, and applied routine dependency updates.

Upgrade: npm i -g openlore@3.3.1 — or openlore update.
