Release v0.4.1.

  • Upgrade from 0.4.0: fix normal plugin-owned same-account native OAuth refresh aborting concurrent already-dispatched managed HTTP streams with COPILOT_PREVIEW_CREDENTIAL_CHANGED.
  • Qualify unchanged account, entitlements and official OAuth-derived endpoint through an exact single serialized credential notification and commit result. Immediately invalidate reusable metadata/prepared proof; subsequent requests resolve fresh native auth and metadata without redirecting a frozen account or replaying the user query.
  • Keep unknown/external/Core-only notifications, duplicate/delayed/failed writes, revocation, cancellation and disposal fail-closed. Provider rejection remains an ordinary native error; this is not blanket invisible recovery or a new retry owner.
  • Synthetic native-adapter and credential-store regressions establish this scope, not attribution of any particular live incident. Retain 0.4.0's original immutable tag/archive; use the new stable patch. No installation, restart or live settings mutation is included.
