Cloudhal 0.3.9

All @cloudhal/* packages and create-cloudhal at 0.3.9.

Changes

• cloudhal domain set <host> --redirect www.<host>: hosts that answer with a 301 to the site's domain (path and query kept), routed to the site Worker as Custom Domains on the next deploy (#49). cloudhal domain remove redirects drops them.
• cloudhal domain set <host> also makes the host the local site's primary domain, so canonical links, the sitemap and feeds use it before the first deploy (#48).
• cloudhal snapshot --publish makes no new snapshot when nothing changed since the live one; it says "nothing changed since " instead (#51).
