this PR adds a temporary test file for River Cities Central Office (trying to help them visualize the switch to TSML UI)
also helps River Cities (and a few other sites I've seen) by inferring the
countryvalue when it's not present but thestateis in the USA or Canadaadds an "Appointment" option to the time dropdown when there are appointment meetings in the data - otherwise they get added to the bottom of the list and are hard to find
