An inline watcher whose host is gone is now orphaned, not healthy.

A Monitor-hosted watcher now notices when nothing is reading it, and steps aside. A field
report from an agent on Grok Build (0.8.24): after a session restart, Grok killed its Monitor
tasks, but the confer arm processes survived and were reparented. For 1h39m watch-status said
healthy, delivery: monitor while nothing read a byte. The next confer arm then refused to
replace them, correctly declining to steal what looked like a live co-resident's watcher. The agent
was dark behind a green status: the inverse of 0.8.31's false death.

A pipe whose reader has closed polls as POLLHUP on macOS and POLLERR on Linux, with no write
needed. An inline watcher checks for this on every loop, and exits and releases its lock once
nothing is reading. A write that fails because the reader vanished mid-wake now exits too, instead
of retrying into a dead pipe. The cursor had not advanced, so that wake is delivered to whoever
arms next.
A new state, orphaned, for watchers too old to exit by themselves. A monitor-delivery watcher
whose parent is now init (or a systemd/launchd subreaper) has no host left to read its output.
It belongs to nobody, so replacing it steals nothing. confer arm replaces it where it used to
leave it alone. That matters on upgrade: on 0.8.33 such an orphan was left running while arm
attached to an empty spool, which looked armed and was still dark. watch-status, the
session-start hook and doctor all report it by name.
