Changes

Query approved remote knowledge without requiring a local checkout; local and non-MCP retrieval remain available.
Bundle the repository-scoped evidence Wasm plugin for read/read_many, with section references and commit-specific source URLs.
Install the plugin during root workspace initialization, or add it to existing workspaces using context evidence install. Unknown or modified plugins are preserved.
Reuse attached evidence and ready source URLs instead of repeating metadata lookups. Registered references remain distinct from verified source reads.
