Search never tells an agent the index is stale or to grep. The API now
keeps each project's default branch indexed from its GitHub remote, with no
machine or checkout involved (contextstream API migration 853), and reports index_trust.canonical.state (current, catching_up, unverified).
Freshness follows that state, not the age of the last write: an index that
holds the remote head is fresh however long ago the last push was, and one
the server is syncing is not stale.
Default search output no longer carries index_health, index_trust or ingested_at_max; they appear with CONTEXTSTREAM_DEBUG=1 and in project(action="index_status"). When the server confirms the index holds
the remote head, search adds index_source (repository, branch, head, current).
No index recommendation names a path any more. A project's recorded path
can belong to a machine that no longer exists; the init(folder_path=...) advice remains only for a folder this process has
just read.
project_index_state is never rewritten to stale from an age.
A checkout on another branch or commit is not flagged [INDEX_TRUST] when
the index follows the remote, and [INDEX_CHECKOUT] notes about other
machines' checkout roots are dropped for such projects.
project(action="index_status") says which remote branch the index
follows and whether it holds its head.
Searching another project of the same workspace no longer raises [SCOPE_MISMATCH]: the server answers it from that project's current
default branch, with no checkout or init. Across workspaces it still warns.
