Fixes

• Resume tooling hand-off is now driven by a deterministic preflight agenda. The dashboard / move-to-editor / extension offers kept getting dropped on resume because step (a) was prose the agent was asked to run before diagnosis, while the preflight blob it reads first carried no tooling agenda — so an open blocker or an obvious resume action pulled attention past the offer. consort-preflight now emits tooling.offers_pending (dashboard + extension always owed and silently cleared when already satisfied; move-to-editor only when outside an IDE terminal), and start.md's step (a) must clear that list before the situation report and before consort-next/diagnosis.
