This PR does two things:
fmt::Debugbounds onHierarchicalLayer, which makes it possible to composeHierarchicalLayerwith otherHierarchicalLayers (Subscriber: Debug #45).HierarchicalLayerhas already placedDatain the extensions; skipping if it's already present. This prevents the panic reported in tracing-subscriber panic when using tracing-appender with tracing-tree tokio-rs/tracing#2112.
