Updates Lodash to 4.18.1 and replaces the unmaintained Istanbul coverage command with NYC. Updates Mocha and Coveralls to their latest releases.

Fixes recursive CLI flags, removal at the end of a file, unknown block handling, replacement lists across files and recursive includes, and directory error reporting. The CommonJS API remains compatible with grunt-processhtml.

Validation: 50 tests pass. Coverage includes the library, file writer, and CLI, with 100% of statements, branches, functions, and lines. The upstream grunt-processhtml suite passes all 14 assertions. Development tools require Node 20.19+ or 22.12+.

The production audit is clean. Eleven development dependency advisories remain, including two critical advisories through Coveralls.
