/flow-auto can complete an inspection review when the declared audit fails,
without treating that failure as a passing product gate.

• An all-inspect plan may declare gate-observe. Flow records a complete failed
  gate as evidence for independent review and reports its exit in delivery.
  Change plans and extra evidence still require passing results. An observed
  gate cannot authorize a prerequisite repair amendment.
• For a fresh, exact-goal review-and-roadmap request, /flow-auto may approve
  its own inspection draft. Plan-only, mixed implementation, ambiguous, and
  pre-existing drafts retain the approval checkpoint.
• Session v5 schema accepts the gate-observe evidence-scope value. Existing plans keep their
  pass-required gate semantics. Active sessions should be finished or closed
  before upgrading.
• The 9.3.0 qualification profile adds a required failed-audit inspection case
  and measures GPT-6 Sol on OpenAI only. It makes no cross-provider reliability
  or delegated Jev qualification claim.
