/flow-auto carries approved tasks through validation, independent review and
bounded same-goal prerequisite repair with clearer evidence and recovery guards.

• Exact supplemental checks distinguish required passes from observations.
  Complete nonzero observations remain visible without clearing a failed required
  gate. Approved prerequisite repair keeps its gate and amendment limits.
• Prerequisite repair guidance preserves test discovery, assertions and
  acceptance criteria. Checkpoint reports offer defer or abandon and distinguish
  historical failed-gate observations from a current gate verdict.
• Independent reviewers receive original-source baselines and bounded context
  and diff pages. Missing or stale evidence stops review preparation before it
  consumes a failed-review attempt.
• Failed inspection reviews now use bounded retry for report/evidence repairs.
  Failed audit observations remain admissible. Previously completed failed inspections
  remain readable with unsupported assurance.
• Automatic retry guards also cover Jev-off runs, generic child mutations and
  stale handbacks. Native automatic compaction survives historical message
  updates. Repeated failed reviews and scope blockers still require direction.
• Jev configuration, actual assessment and recovery authority remain distinct.
  A missing key leaves advice off; configured shadow advice grants no mutation.
  Delegated Jev recovery has no qualified release profile.
• Session v5 schema adds optional baseline/evidence references and exact feature
  checks. Existing documents remain readable; finish or close work before
  downgrading to an older strict reader. No persisted continuation state is added.
• The 9.4.0 qualification profile adds three autonomous cases with native lineage,
  complete reviewer access and original packet bytes. It pins OpenAI GPT-6 Sol on
  OpenCode 1.18.31. This profile requires a live matrix and exact-artifact
  canary. Local controls do not establish model quality, cross-provider
  reliability or delegated Jev qualification.
