Fixed

• Leave @oxc-project/runtime helper imports to OXC.bundle/2, which provides them. Since 0.19.1, builds whose transformed code used a helper, such as lowered class fields in Svelte islands, failed with {:not_found, "@oxc-project/runtime/..."}.
• Resolve application sources from their configured package scopes and node_modules before the directories above them. Since 0.19.1, a node_modules higher up could supply a second copy of a framework runtime. Packages still resolve their own dependencies from the nearest node_modules first.
