⚠ BREAKING CHANGES

• PayloadConverterExtension is renamed PayloadConverter, and
  the former PayloadConverter function type (item, toKind) => Item is removed
  in favour of convertFunc, (item) => Item. identityPayloadConverter is now a
  function of the payload kind returning a PayloadConverter, and the fourth
  argument of adaptReadableStream is a PayloadConverter. Deep imports of
  source files must use the new folders.
