• Detects an assistant tool_use with input: {} when a later same-file tool_result for that ID has is_error: true.
• Keeps transcript bodies, tool names, IDs, arguments, and error text out of output.
• Correlates only within one file, groups metadata across split assistant rows, and fails closed on malformed or duplicate correlation fields.
• Preserves the existing missing-tool-use-block detector and exit-code contract.
• Tested on Python 3.9 and 3.13 across Linux and macOS.
