Added

• batch() on the PostgreSQL adapter, with batch in its capabilities: the operations run in one transaction, each under a savepoint, and the batch goes on past one that fails, as the better-sqlite3 adapter's does. The result has the same shape, a failure of the transaction itself included (index -1). A transaction handle's batch() runs in that transaction; a held connection's runs in a transaction of its own.
• registerFunction(name, fn, { deterministic }) on the better-sqlite3, sql.js and IndexedDB adapters: a JavaScript function callable from SQL. The sql.js and IndexedDB adapters register it again each time sql.js reopens the connection (a save, an export, an import), which drops it.
• ensureColumnExists(db, table, column, definition), named under 0.5.2 below and missing from the package since: it adds a column when the table lacks it, on PostgreSQL and SQLite, and says whether it did.
