feat: Upgrade otel dependencies

Notable bumps:

• @opentelemetry/instrumentation-pg@0.47.0 - fix possible memory leak issue
• @opentelemetry/instrumentation-aws-lambda@0.46.0 - record lambda cold start times as span attribute

Otherwise generically update "@opentelemetry/*" dependencies to 1.27.0/0.54.0

So it seems this is failing because jest doesn't like the nested import:
