v0.3.2-alpha — External review fixes

What's Changed

Fixes for 5 issues found during an independent review of the proxy API:

1. Content-Type tolerance — /api/* (Ollama-native) endpoints now accept requests without Content-Type: application/json, matching real Ollama behavior
2. Missing model → 400 — model is now a required field; returns 400 Field required instead of silently falling back to root model
3. No more URL leaks — Error responses no longer expose internal infrastructure URLs (e.g. http://localhost:11434/...)
4. Actual token counts — Passthrough responses now include real prompt_tokens / completion_tokens from Ollama instead of 0
5. Config endpoint fix — Removed stale root_provider_url reference that caused /v1/rlm/config to 500
