Storify Daily Entry (2026-10-23)
From 2026-10-22 07:58 to 2026-10-23 07:58 UTC, the clearest change came from a person: a maintainer asked for review of a checkout repair, five reviewer workflows ran successfully, and the PR merged. Elsewhere, agents often reached their work but stumbled when publishing it. The day’s recurring question is whether completed analysis becomes a durable issue, comment, or PR update.
The two collected slices contain 524 run records. Their run collection has no remaining cursor, but episode lineage is partial because some recorded parent and root runs were unavailable. The links below support the observed transitions; they do not prove a common cause for similarly named failures.
Episode Highlights
A checkout repair met a full review fan-out. At 19:39 UTC, pelikhan requested
/reviewon PR #71210. Design Decision Gate, Matt Pocock Skills Reviewer, Ponytail Reviewer, PR Code Quality Reviewer, and Test Quality Sentinel launched within seconds; all five runs completed successfully. The PR replaces branch-name fetching with the retained PR head ref to address checkout after a merged source branch disappears. An automated review approved the fix, and PR #71210 merged at 20:02 UTC. This is a strong review-and-merge outcome; production behavior after merge remains unverified. See §39842124572.Work-queue agents repeatedly reached a publication boundary. An earlier experimental work-queue run, §39757480430, failed both agent conformance and conclusion issue projection, so it cannot be counted as an agent-success case. Later, two eslint-monster dispatches and one eslint-miner dispatch had successful agent jobs but failed the conclusion step “Project this run's work queue Issues”; §39896393918 is representative. The observed result is stalled issue projection, with no verified recovery in this window.
Other work also stopped at hand-off steps. Three distinct workflows—compiler surfacing, Smoke Project, and Smoke Claude—had successful agent jobs before “Process Safe Outputs” failed. The compiler-surfacing run also failed ledger reconciliation; §39864188905 shows both boundaries. Separately, create and update cross-repository PR smoke runs failed while checking out
github/gh-aw-side-repo, before their agents could do the intended work (§39880055826). These symptoms are distinct; the evidence does not establish one shared root cause.Feedback Loops Across Workflows
The PR review fan-out loop looks improved for the reviewed checkout repair: one human command produced five successful reviews and a merge. Its longer-term effect is still open. The publication-boundary repair loop remains stalled: successful agents in several workflows were followed by failed issue projection or safe-output processing. The cross-repository smoke validation loop also remains stalled at checkout in both create and update paths.
Run-level boundaries and confidence
failure-83ad4d9554829948marks issue projection in late eslint-monster runs 39896371180 and 39896393918; eslint-miner run 39896417245 has the same named step. An earlier run 39757480430 also failed agent conformance. Confidence high for job boundaries, low for a shared cause.failure-f3527e2bef0656a6covers three unique runs 39864188905, 39876730139, and 39877179050. Confidence high for the failed step, low for a shared cause.failure-db6478627c51efa9covers two unique runs 39878492349 and 39880055826. Confidence high for the failed checkout step, low for its underlying cause.Human Interventions That Mattered
The maintainer's
/reviewcommand converted PR #71210 into a five-workflow review episode. The merge then moved the retained-ref checkout fix into the repository. Automated review feedback called out minor follow-ups while approving the repair; no human intervention was found that resolved the publication or side-repository checkout failures within this window.Signals to Watch Next
Watch for a later reviewer run against a deleted source branch to validate the checkout fix in production; a successful work-queue issue projection after the eslint dispatches; and a cross-repository PR smoke run that clears the side-repository checkout step.
