The 24-hour slice captured 373 workflow runs between 2026-10-23 07:58 and 2026-10-24 07:53 UTC. Its strongest story is a work-queue check becoming an operational test: conformance workers exposed settlement failures in the morning, a newly merged daily smoke workflow reached the same trusted-control boundary in the afternoon, and a maintainer redirected the repair toward automatic first-use pool installation.
A second, shorter episode crossed eight unrelated workflows: each stopped in activation while restoring daily AIC scan observations. A later Daily Go Test Parallelizer run completed, but the cause of the earlier interruption is still unknown. The summary has no canonical episode edges, so the conformance and smoke runs are connected by workflow purpose and timing rather than a proven parent-child chain.
Episode Highlights
Conformance workers stalled at settlement. The Engine Conformance Dispatcher §39932309248 failed in
safe_outputs / Process trusted work queue controls. The Claude and Codex workers failed claim reconciliation and later conclusion steps; the Codex example is §39934255576. The Agy worker §39934288281 also failed agent assertions, so its failure should not be attributed solely to queue settlement. Outcome: stalled; high confidence in the failed steps, lower confidence in a common cause. The new smoke path caught a deployment gap. Maintainer PR #71363 merged at 13:42 UTC. At 14:38, the scheduled Smoke Work Queue Dispatcher §39971310352 failed trusted work-queue controls, and its downstream verifier failed to confirm dispatch. Follow-up #71495 identifies an absent
smoke-work-queuepool in installed Policy despite its presence in source configuration. Outcome: diagnosis and repair in review; no verified live recovery by the cutoff. The missing-pool explanation comes from the PR investigation, while the run independently confirms the failing boundary. An activation interruption spread, then cleared in one observed path. Eight unique runs across AB Testing Advisor, Avenger, AW Failure Investigator, Copilot PR NLP Analysis, Daily Go Test Parallelizer, Feature Grower, Glossary Maintainer, and Issue Monster failed
activation / Restore daily AIC scan observationsbetween 10:03 and 10:33 UTC. In §39939907542, the agent job never ran. The same Daily Go workflow later completed activation, agent, safe outputs, and conclusion in §39955435424. Outcome: recovery observed for that workflow; cause and fleet-wide recovery remain unproven.Feedback Loops Across Workflows
The Publication-boundary repair loop remains unresolved. Conformance failures at claim reconciliation and Issue projection, the smoke trusted-control failure, and later projection failures in Daily Token Consumption Report §39957736571 and ESLint Monster §40059714295 keep feeding diagnostics back into queue repair. These are distinct failed steps, not proof of one defect.
The Smoke-evidence validation loop is improving in review. The merged smoke workflow made a missing installed pool visible; PR #71495 then drew a concrete maintainer correction and multiple review rounds. Its operational trajectory remains unverified until a later smoke run passes. The AIC burst is an incident to watch, not yet an established repair loop.
Human Interventions That Mattered
A maintainer merged #71363, putting the daily smoke check into the observed run path. On #71495 at 21:13 UTC, the maintainer rejected administrator-only seeding and requested automatic pool installation on first use. The agent revised the proposal accordingly; later reviewers requested further changes, and #71495 remained open at the cutoff. That changed the proposed recovery path, but the logs do not show a deployed fix.
Signals to Watch Next
Watch the first smoke dispatch after #71495 lands for a verified worker launch, and check whether claim reconciliation and Issue projection stop failing across queue consumers. Also watch whether
Restore daily AIC scan observationsrecurs; the observed later success does not explain the earlier eight-run burst.Evidence and coverage notes
The pre-downloaded summary contained 373 runs, 361 executed runs, and 325 executed successes, with no continuation cursor. The eight AIC activation failures are one unique-run cluster (
failure-ccf6bfe72ce3cbab); overlapping work-queue clusters were not added together. Relevant boundaries includefailure-61402b9699b22252(trusted controls),failure-f9c73906a5c3ae0e(claim reconciliation), andfailure-83ad4d9554829948(Issue projection). Cached run summaries and MCP audits were checked for the representative runs. The summary reported zero canonical episodes and edges, limiting causal lineage claims.References: §39934255576 · §39971310352 · §39939907542
