Across the 24 hours ending 2026-10-26 08:06 UTC, the work queue produced a morning report, then faltered at later publication and handoff points. In parallel, a retry telemetry pull request drew several automated reviews and direct maintainer guidance; its follow-up smoke run exposed another failure before the new telemetry could be validated.
The two log segments contain metadata for 449 runs. I audited representative runs and checked their job and step conclusions against GitHub activity. Storage pruning left detailed local summaries for 392 runs, so the episode links below are representative rather than an exhaustive causal map.
Episode Highlights
A work queue report gave way to failed dispatches. Daily Work Queues Report completed, but the Daily Discussion Report Dispatcher and Smoke Work Queue Dispatcher later failed at Process trusted work queue controls. The smoke run also failed its verified dispatch check. A later formal verification run failed uploading its analysis handoff. These are distinct job boundaries; the audits do not establish one shared root cause. Confidence: high on the observed failures, low on a common explanation.
Review feedback moved retry telemetry forward, while smoke remained red. PR #71934, opened for issue #71824, received a successful skills review with inline findings and a successful PR Sous Chef coordination run. The maintainer asked for unified session integration, conflict resolution, and then
/smoke-copilot. The resulting smoke run failed in the agent job at Docker image build and session contract checks. The PR remained open at the window’s end; the smoke failure does not prove the telemetry change itself is defective. Confidence: high on chronology and run outcome. The container scan kept its issue ledger current but did not clear its gate. The daily scan published eight issue updates, including the CVE burn-down issue, then failed at Enforce vendored vulnerability and license gates. The issue remained open. Confidence: high on the published updates and failed gate; no verified remediation appears in this window.
Feedback Loops Across Workflows
The Publication-boundary repair loop remains mixed: a successful work queue report was followed by two control-processing failures and a separate formal verification handoff failure. Repeated checks are making boundaries visible, but the evidence does not yet show sustained recovery.
The Smoke-evidence validation loop is active: reviewer findings and maintainer comments drove revisions to PR #71934, and an explicit smoke dispatch tested the branch. The smoke result is still failing, so the loop is producing feedback rather than a validated fix.
Human Interventions That Mattered
On PR #71934, the maintainer explicitly requested unified session integration, merge conflict resolution, and the smoke run. Those comments changed the work sequence and led to a concrete test. Automated review findings and PR Sous Chef coordination supplied the detailed follow-up list. No comparable human pivot is visible for the work queue or scan episodes in the inspected evidence.
Signals to Watch Next
Watch the next trusted work queue control and formal verification handoff results, the next PR #71934 smoke run after its follow-up commits, and whether the container scan gate clears while its issue ledger stays current.
