Storify Daily Entry (2026-10-28)
In the 24 hours ending 2026-10-28 07:57 UTC, the collected logs covered 524 runs after following both continuation cursors. The clearest story began around PR #72415: a report-tree refactor drew detailed review, a linked dispatch swept twelve smoke workflows, and the author made targeted fixes before merging. The sweep exposed four failures across distinct engine or setup boundaries, so its outcome was mixed even as the review loop moved the code forward.
A second pattern persisted after agent work completed: PR Sous Chef and ESLint Miner each had runs whose agent job succeeded but whose Process Safe Outputs step failed. That is a shared failure boundary, not evidence of one shared cause. Other scheduled failures were visible, but no causal connection to the PR or publication failures was established.
Episode Highlights
Review → repair → smoke sweep. The recorded dispatch lineage connects twelve smoke workflows to PR #72415. Smoke Agy §40522202506 failed in its experimental CLI and conformance steps; Smoke Copilot Small §40522256045 failed during browser installation and session verification. Two AOAI smoke agent jobs also failed, while the other eight linked runs had no failed jobs. Review comments identified publication, mention, managed-memory, and type-directive regressions; the author replied with fixes in commits 6fd9e4e and 675eaa6. The PR merged at 15:23 UTC. Outcome: review repair landed; the post-merge result at each previously failing smoke boundary is unverified. Confidence: high for lineage and review response, lower for regression closure.
Publication stalls across workflows. In PR Sous Chef §40576047723 and ESLint Miner §40628348440, the agent job succeeded and the safe-output job failed at Process Safe Outputs. The PR Sous Chef audit records add-comment and create-issue calls. Later PR Sous Chef samples still failed at that step through 07:37 UTC. Outcome: stalled. Confidence: high for the job boundary; low for any shared root cause or link to PR #72415.
Separate scheduled stalls. Windows Grower §40480547110 failed while downloading container images; Daily Caveman Optimizer §40580344835 failed in agent execution and step-summary parsing. These are distinct observed failures. Outcome: recovery was not established in this window. Confidence: high for failed steps, low for any relationship between them.
Feedback Loops Across Workflows
The Smoke-evidence validation loop moved from review findings to fixes and broad smoke evidence. Its direction is mixed: the PR merged, but four linked smoke runs failed and later success at those boundaries remains unknown.
The Publication-boundary repair loop remains unresolved. Repeated PR Sous Chef failures and an ESLint Miner work-queue failure reached the same safe-output step after successful agent jobs. The recurring boundary reinforces the need to inspect publication outcomes; the sampled logs do not prove a common defect.
Human Interventions That Mattered
On PR #72415, reviewers raised concrete issues in the report-tree change, and the author responded with specific fixes before merging. Those actions changed the code under review. I found no comparable human intervention that demonstrably resolved the publication or scheduled failures during this window.
Signals to Watch Next
Watch post-merge smoke runs at the four failed boundaries; whether PR Sous Chef and ESLint Miner stop failing in safe-output processing; and whether Windows Grower and Daily Caveman Optimizer recover on their next schedules. The parent smoke dispatcher was outside the stored lineage sample, and broad PR search was unavailable, so other intervention links may be missing.
Audit and coverage notes
Representative agentic-workflows audits inspected §40522202506, §40576047723, §40628348440, §40480547110, and §40580344835. Collection returned 400 initial runs and 124 continuation runs; the final continuation found zero more. Failure clusters overlap, so no cluster counts were added together. The inspected failures are representative, not proof of a fleet-wide cause.
