The day opened with a repeating publication failure and then shifted into a human-triggered, cross-engine smoke sweep. Across 480 collected run records for 2026-10-28 07:55:49 to 2026-10-29 07:55:49 UTC, the clearest chain joined one PR command to 12 smoke workflows. Nine succeeded and three failed. Later, a separate scheduled smoke run reached the same publication step where PR Sous Chef had been stalling. These are observed patterns, not a proven common defect.
The strongest human signal came from PR #72543: review comments at 12:53 UTC identified two concrete gaps, and the author answered with commit
05f99d4216at 13:16 UTC. The PR remained open at the time of review, so the effect of that revision on the earlier smoke failures is still unknown.Episode Highlights
One command, twelve engines. The recorded lineage from command run §40687413135 fans out across Agy, Claude, Codex, Copilot, OTEL, and Pi smoke workflows. Nine runs passed. Agy §40687536087 failed in agent execution and conformance assertions; two AOAI variants also failed. The cached lineage lacks its parent run, so the 12 children are a supported observed set, not a complete account of every possible dispatch. Confidence: high for the recorded fan-out and outcomes; medium for its completeness.
Publication kept failing after agent work. Fifteen PR Sous Chef schedules in the added early interval all failed. In representative §40675051360, agent and detection jobs succeeded, an issue was declared, and
safe_outputs / Process Safe Outputsfailed. Later Smoke Project §40792230830 also had a successful agent job followed by that failed step. The matching boundary does not identify a shared cause. Confidence: high for the job sequence; low for a common cause. Two scheduled engines stopped earlier. Daily Caveman Optimizer §40765587560 failed at
Execute Claude Code CLI; Smoke Azure Codex §40791835794 failed atExecute Codex CLI. Both then entered failure handling. These are separate execution stalls, with no demonstrated link to the publication failures. Confidence: high for the failed steps; low for a shared explanation.Feedback Loops Across Workflows
The Smoke-evidence validation loop moved from a PR command to cross-engine evidence, then to reviewer comments and an author revision. Its direction is mixed: the sweep exposed three failures, and the later revision has no verified post-change run here. The Publication-boundary repair loop remains unresolved: repeated PR Sous Chef schedules and a later Smoke Project run reached the same failed processing step, but the audits establish symptoms rather than a repair or root cause.
Human Interventions That Mattered
On PR #72543, the author explicitly accepted review feedback and added regressions in
05f99d4216, changing the implementation after the first smoke sweep. Separately, PR #72651 was merged at 11:31 UTC with fixes for workflow feature and race CI regressions, including the Windows grower caller permission. That merge is a concrete maintenance pivot; this sample does not prove downstream recovery.Signals to Watch Next
Watch post-revision smoke runs for the three failing variants, later PR Sous Chef and Smoke Project publication outcomes, and the next Caveman and Azure Codex schedules. The 59-run continuation closed the collection cursor; representative audits and PR reads were selective, so causal conclusions remain limited.
