The day’s clearest pattern was work reaching its final handoff and failing there. Across the 24-hour window ending 2026-11-01 07:56 UTC, the collected reports contain 605 distinct runs. In two disjoint segments, 129 distinct failed runs reached the
safe_outputs / Process Safe Outputsboundary. The early segment was PR Sous Chef; later runs also included Smoke Project and ESLint Refiner. Representative agent jobs had succeeded, so the visible loss was in publication. The shared step identifies where these runs failed, not a shared root cause.A second story was a human-guided review cycle on PR #73672. Several review workflows commented; Peli accepted specific findings and recorded follow-up commits. That PR remained open at the window end. Its Git-generation hardening has no demonstrated causal link to the publication failures above.
Episode Highlights
Publication handoff kept breaking. PR Sous Chef’s §41223689503 belongs to the 53-run early failed-step cluster. Later, Smoke Project §41255576901, ESLint Refiner §41269711804, and PR Sous Chef §41285731195 show the same failed step in a 76-run cluster. In those later representatives, the agent job completed and safe-output tool calls were recorded before processing failed. Outcome: stalled; confidence is high for the boundary and recurrence, low for a common cause.
Review findings became revisions. Matt Pocock Skills Reviewer §41262184930 succeeded and submitted two inline findings on PR #73672. Peli’s reply on diagnostic matching and reply on the legacy token option cite commit
0ba7c3ffff; later replies cite93bc203cd0for further changes. Outcome: improved inside the open PR; deployment and runtime effect remain unverified.Work-queue dispatches exposed different boundaries. An Aider conformance worker §41189147646 failed during
Execute Aider CLIandAssert engine conformance, then its conclusion job also failed. A later ESLint Refiner worker §41269711804 completed its agent job but failed in safe-output processing and issue projection. Outcome: stalled for separate observed reasons; the reports do not establish a shared cause or a direct parent-child lineage between these workers.Pi smoke checks repeated a runtime signal. Smoke Azure PI §41255254996, Smoke Pi §41256713068, and Smoke Pi Auto §41256886716 each failed at
agent / Execute Pi CLI. The subsequentHandle agent failurestep is aftermath. Outcome: unresolved in this sample; the exact driver cause is unknown.Feedback Loops Across Workflows
The publication-boundary repair loop remains stalled: successful agent work repeatedly reaches a failing publishing step across three workflows. This continues the label used in prior Storify entries; no linked repair was verified this day. The review-gated design loop moved forward on PR #73672 as review findings led to human revisions, but remains open. A runtime smoke feedback loop emerged across three Pi variants, with repeated driver-step failures and no observed recovery.
Human Interventions That Mattered
Peli distinguished actionable inline review findings from completion-only notices on PR #73672, accepted changes, and documented their commits. This narrowed the review loop to concrete revisions without treating a no-op reviewer status as validation. Peli also merged PR #73578 on universal-consumer credential routing during the window; the sampled failures do not demonstrate that this merge resolved the publication or Pi signals.
Signals to Watch Next
Watch whether
Process Safe Outputssucceeds again in PR Sous Chef, Smoke Project, and ESLint Refiner; whether PR #73672 merges and its behavior is validated after deployment; and whether the Pi smoke variants recover. The continuation query reached no further cursor, but these are sampled run and review observations, not proof of complete episode lineage or root cause.References: §41285731195 · §41269711804 · §41262184930
