gopls: attach logs to gopls bug report

We currently run gopls bug to report gopls issues automatically. We should attach logs or a stack trace too.

For now, we are not automatically appending the logs - since they may have sensitive information such as private repo code paths. Instead adjusted the template so it is more obvious to users that they need to fill in details and how to do that.
