AI merge improvements + header UI fix

Syntax highlighting: Vendored highlight.js 11.10.0 with atom-one-dark theme. Merged file previews are syntax-highlighted — INI for most /etc config files, bash for shell configs, nginx, xml, yaml as appropriate.

Summary bullets: The merge prompt now asks the AI for a structured response with a SUMMARY section (bullet points of what was kept/added/changed) followed by the MERGED file content. Parsed server-side on a SUMMARY:/MERGED: delimiter with fallback if the model ignores the format.

AI buttons in header: Explain and Merge buttons now sit in the card header row, visible before expanding the diff. AI result panel is always visible regardless of whether the diff is open.
