Fixed

• HF readers stopped reading cards after a cold boot (card ignored at the
  Windows logon screen while the service reported itself as running). The
  HF loop now watches every PC/SC reader, follows readers being attached or
  removed, and recovers from any PC/SC error instead of stopping silently.
• Requests to the Ironchip API now time out after 30s; a stuck request used
  to block every card read.
• A config file that could not be read at startup (e.g. held by an
  antivirus) was overwritten with defaults, wiping the API key.
• An unplugged HID reader no longer respawns the session helper every 2s.
• configure no longer resets the log level to info on every run.

Changed

• MSI upgrades keep the installed API URL, API key and reader mode, also on
  silent upgrades without API_KEY.
• Builds sharing the installed version now upgrade it instead of installing
  side by side.
• The service restarts automatically if its process dies.
• The API key no longer appears in verbose MSI logs, and the installer no
  longer writes C:\ironchip_ca.log.
• Reader names are logged when HF readers become available or change.
