Fixed

• BUG-6: `--out` / `--output-dir` permission denied now exits 1 cleanly

  Two paths in `pipeline.py` had unguarded file writes:

  • Main `--out` handler: `os.makedirs(output_dir)` moved inside the `try/except`
    block so PermissionError on directory creation is caught and reported cleanly.

  • `--watch-check --output-dir` handler: `makedirs` + `open()` now wrapped in
    `try/except Exception` — prints `"could not write output file: …"` to stderr
    and exits 1, consistent with the main pipeline path.

  Regression originally from v1.2.1, crept back in v2.1.9.
