Added

• indexedExtensions setting (default ["php"]): drives the file-operation registration globs and the watched-files glob instead of a hardcoded *.php. Rename refresh requests are only sent when a rename touches an indexed extension.
• PHP 8.6 target version: accepted as a target, so partial-application placeholders no longer report a syntax error. The default stays 8.5.
• Real parameter defaults: signature help and hover show the actual default values of indexed callables.

Changed

• Navigation resolved by mir: go-to-definition, go-to-declaration (including method overrides), type definition, hover class lookup, declaration name ranges, type-hierarchy supertypes/subtypes (including trait users) and outgoing calls now come from mir instead of AST walks and name matching. new Foo is no longer listed as an outgoing call.
• Call hierarchy by position: prepare picks the method under the cursor, and outgoing calls end on the declaration mir resolved rather than the first of that name.
• PSR-0 fallback removed: mir's autoload map already resolves PSR-0, including namespaced Foo_Bar classes the duplicate resolver mapped to the wrong path.
• Signature help, inlay hints and resolve handlers run on a snapshot: they no longer hold the session lock for the whole request, so edits are not blocked.

Fixed

• Member completion: interface members (including parent interfaces) are offered, inherited private members and protected members outside a class are hidden, parent/trait/mixin names resolve in the declaring file's namespace, and FQCN receivers match by short name. Built-in class methods keep their real case (getMessage), and enum completions are no longer duplicated.
• Namespace-aware lookups: qualified class names no longer match same-named classes in other namespaces in member and parameter lookups.
• Hover: a use alias no longer resolves to a same-named enclosing declaration; hover survives close and reopen.
• Goto-definition: bare function calls resolve by FQN, and member calls on foreign receivers skip the same-file name match.
• Document lifecycle: notifications are ordered, and lifecycle diagnostics are versioned.
• Rename: declaration lookup is case-insensitive.
• Positions past EOF are clamped to the end of the source.
• Analysis locks: a panicked analysis no longer poisons that file's in-flight lock, and the mention scan reads off the session lock.
• Stale diagnostics survive the initial workspace scan: a file opened while indexing was still running was analyzed against a partial index, and the analysis cache — keyed on (source, decl_version) — was never invalidated when the scan finished, so the post-index republish re-served the same memo. Symbols declared in files the scan had not yet reached stayed reported as undefined until the file was edited. mark_index_ready now bumps decl_version, the same invalidation note_new_file_declarations performs on the didChangeWatchedFiles path.
