Re-organize the examples directory

We'd like to re-organize the examples directory slightly to simplify the scripts there and better allow future contribution.

• Add a toplevel README describing the requirements for an example.
• Move the general tools (word piece vocab script), out of the bert directory.
• Simplify file names.
• Move most configuration into python files (no json).
