[bukawa] Delete legacy and add release source updates

• Deleted legacy/b/bukawa folder and added new source files to release/b/bukawa
• Attempted to reconstruct the HISTORY.md
• Tidied up documentation files to be more consistent and not use Word html (merged contents of welcome.htm and Bukawa_Kbd_table.htm into welcome.htm.
• Added a bukawa.php file

@gawabill I did update some of the files to better our structure. Primarily I merged two of your files into welcome.htm so there would only be one keyboarding description. Some of it does duplicate, but you can choose how to update that in the future. I'll attach the files as they are in the repo so you can start with these in the future.

Thanks Lorna, for your help with this. Just for my edification, how would I go about revising/updating any of the support files for the keyboard. Would I take the one from GitHub and then after any changes, issue a Pull Request?
