Sharing an options map between two simultaneous uses of GPUdbSqlIterator
would point both at one server-side result table. Two statements running
at the same time raced to create it and one would fail; where they did not
overlap, either could return the other's rows instead of its own, with no
error. The two also competed when cleaning the table up afterwards. There
is now no contention. A query whose result is read in a single page and
non-simultaneous uses of GPUdbSqlIterator were unaffected.
GPUdbSqlIterator dropped each of its server-side tables once per page
read rather than once, so a long iteration closed with hundreds of
redundant requests.
