Codex failure reports now describe the latest operation, expected result, observed failure, and hook response using safe fixed vocabulary in the existing report message.

The original fingerprint, count, severity, and acknowledgement semantics remain unchanged. User impact is explicitly unknown and the report is labelled diagnostic_only. Earlier occurrences are not characterized by the latest description. Raw errors, paths, prompts, and session identifiers remain outside reports.

This improves future reporting. It does not claim recovery of the withdrawn Windows incident or native PreCompact validation.
