Hook modernization for Claude Code SDK 2.1.195. All changes are backward-compatible and presence-guarded -- zero behavior regression.

• hooks: session-start now logs permission_mode for audit and bounds its additionalContext to ~9K (10K disk-spill safety threshold, CC 2.1.174). pre-task skips KNOWLEDGE injection when permission_mode === 'plan'. forced-eval bounds its additionalContext to ~9K.

Fixed

• post-task: hardened subagent-failure detection to read tool_result ?? tool_response -- the legacy field stays primary (zero regression) while future-proofing against the documented SDK field rename.

• hooks: session-start logs permission_mode. manager-prompt bounds injected additionalContext to ~9K at both output sites (codeword detection + HARD-wall logic UNCHANGED). hardmode-guard re-verified on 2026-06-27 that the agent_id/agent_type main-vs-subagent discriminator still HOLDS on CC 2.1.195 (defensive comment added).
