Options to specify percent or amount dec in financial aid approval module

Right now it assumes that we always want to give full financial aid (100%) when we approve a financial aid grant, but it's possible this isn't always desired (or feasible) for a program/chapter. We should add two fields to the approval form that allow the admin to specify the percent and/or amount dec to include in the grant approval.
