- pyproject.toml: raise setuptools floor to 77.0.1 so PEP 639
  license/license-files metadata parses correctly on a fresh build
  (pip install -e . was failing on setuptools 68.2.2-76.1.0).
- mcp/proxy.py + mcp/health.py: recover from a terminated backend
  session on HTTP 404 (not just 400), per MCP Streamable HTTP spec.
