Release 0.16

I'm planning to do release at next week - main reason set_mode bugfix.

I wait #423 and #402 testing results.

Fixes:

• MAVROS sends DO_SET_MODE with armed flag not based on current arming state #423 set_mode bug
• UDPROS still do not work with rospy #394 fix rospy clients of mavlink/to and from topics
• Mavros library depends on mavros_msgs headers #391 catkin_make build error (catkin build are fine)

Changes:

• Remove APM statustext message qurks  #417 remove STATUSTEXT quirk
• Deprecate WaypointGOTO service #414 remove ~mission/goto service
• Remove cmd/guided_enable service. #407 remove ~cmd/guided_enable
• Publish extended state to ROS #395 add VTOL state publisher

New:

• expose local_position_setpoint bitfield #402 setpoint_raw

I tested using PX4 SITL, but i got some error - jMAVSim copter wont fly.
Anyway i checked ARMing and setpoint loopback - works. So i can do release.

http://dev.px4.io/simulation-sitl.html - i do not have nsh prompt and any output. Perhaps ninja eats it?

But mainapp and jmavsim running.
